What is Polymyxin B And Colistin (Colistin sulphate and Coly-mycin S)?

They are active against most gram negative bacteria, notably Pseudomonas; exceptions are Proteus, Serratia and Neisseria. Both have very similar range of activity, but colistin is more potent on pseudomonas, Salmonella and shigella. Little or no absorption occurs from oral route or even from denuded skin (burn, ulcers). Colistin sulfate and colistimethate sodium may both be given intravenously, but the dosing is complicated. Colistimethate sodium manufactured by Axellia (Colomycin injection) is prescribed in international units, but colistimethate sodium manufactured by Parkdale Pharmaceuticals (Coly-Mycin M Parenteral) is prescribed in milligrams of colistin base. This drug is commonly sold as Brand Name Colistin sulphate.

Indication/Contraindication for use of Polymyxin B And Colistin (Colistin sulphate and Coly-mycin S)

  • Systemic urinary tract, meningeal local and enteric infections caused by gram negative organisms.
  • Hypersensitivity.

  • Nephrotoxicity, neurological disturbances, ototoxicity.
  • Intra muscular injection causes pain, flushing and paresthesias after injection.

Dosage and Prescribing Information for Polymyxin B And Colistin (Colistin sulphate and Coly-mycin S)

Polymyxin B; 15,000-25,000 u./kg body wt daily for 7-10 days. Max: 2,000,000 i.u. per day. Colistin Sulphate: 25-100 mg 3 times orally daily children and infants: 515 mg/kg body wt in 3 divided doses.

Drug Interactions of Polymyxin B And Colistin (Colistin sulphate and Coly-mycin S)

Potentiates nephrotoxicityproduced by cephalothin and aminoglycosides. Neuromuscular blockade potentiated by ether anaesthesia, sedatives, muscle relaxants and aminoglycoside antibiotics. Synergy with trimethoprim and useful combination with rifampicin COLISTIN Potentiates action of curariform muscle relaxants, nephrotoxicity of aminoglycosides. Potentiates chloramphenicol, erythromycin and trimethoprim.

Precautions while prescribing Polymyxin B And Colistin (Colistin sulphate and Coly-mycin S)

  • Perforated tympanic membrane, monitor renal functions.
  • Elderly: Use with caution.
  • Lactation: Contraindicated.
  • Paediatrics Use with caution.
  • Pregnancy: Contraindicated.
